We do not provide medical advice, interpretation of results, or treatment recommendations. The monitoring data must be reviewed and interpreted by the ordering healthcare provider. This service is self-pay; insurance is not billed.
Patient Responsibility: Patient agree to handle the ABPM device with care and follow the instructions provided. Patient is responsible for any loss, theft, or damage to the device while it is in patient's possession. In the event the device is lost, stolen, or damaged beyond repair, patient agree to pay the replacement cost based on the current market value of a comparable ABPM device. Repairable damage will be charged at the actual repair cost.
Rental Period and Fees: The ABPM rental fee is $180 for 1 day, $240 for 2 days, or $300 for 3 days. Late returns may incur additional rental charges.
Consent and Agreement: By signing below, you confirm that you have read and understood the terms above and agree to comply with them. You accept responsibility for the safe use and timely return of the ABPM device provided to you by Spkit Diagnostics.
